Brief History

In 1971, Bausch & Lomb introduced the first commercially available soft contact lens. For years, this conventional soft daily wear lens was the only type of soft contact lens available. This lens ideally was meant to last between 6 and 12 months and required daily cleaning and weekly enzymatic treatment.

In 1981 the FDA approved the first contact lenses approved for extended or overnight wear.

It wasn?t until 1991 that the first frequent-replacement contact lenses were sold. Frequent-replacement contacts are typically replaced every 1 to 3 months. The next year, 1992, disposable contacts (disposed every 2 weeks or less) were introduced.

In less than a decade, frequent-replacement and disposable lenses became the lenses of choice for the vast majority of eye care providers. Today approximately 75% of soft contact lens patients are wearing some sort of frequent-replacement or disposable lens. Frequent-replacement and disposable contacts are available as both daily and extended wear.

Soft Contact Lens Options

As implied, daily wear contacts are removed and cleaned daily, while extended wear lenses can typically be worn continuously for up to 7 days (or more as CibaVision?s Focus? Night & Day? contacts have been approved for up to 30 days continuous wear).

Disposable contacts are, as the name implies, disposed on a regular period, while the non-disposable contacts are cleaned and disinfected before reinserting the contacts.

Patients who choose extended wear should be aware of the added risk of eye infections and complications that come with sleeping in contacts. I typically discourage extended wear, but will fit contacts as extended wear only after the patient understands the added risk and agrees to be seen for more frequent follow-ups. I also will use a silicone hydrogel lens material for these patients.

Types of Soft Contact Lenses Available

Soft contact lenses are available for a variety of visual corrections.

For patients with significant astigmatism, I will often use a soft toric contact lens. Toric contacts have improved greatly over the last few years. Indeed, many patients who have never been able to wear contacts due to their astigmatic prescription are now able to successfully wear contact lenses.

Soft bifocal contacts are a relatively recent option for patients requiring a reading prescription or bifocal. Along these same lines, monovision continues to be a good option for these same patients. Monovision correction does not use a bifocal contact, but rather uses one contact for the distance vision (usually the dominant eye) and one contact for the near vision.

Soft contact lenses come as either clear or with a visibility (handling) tint. The visibility generally has no effect on eye color, but is there to help you see the lens while you?re handling it. These lenses are not the same as cosmetic soft contacts.

Cosmetic soft contacts have been popular sellers since their introduction. Now patients with or without a prescription can change the color of their eyes with colored or tinted contacts.

CibaVision?s Wild Eyes? contact lenses and CooperVision?s Crazy Lenses are novelty lenses that are fun to fit around Halloween.

Concluding Thoughts

Part of the reason for the success of soft contact lenses is the comfort these lenses offer over rigid gas permeable (RGP) contacts. As a result, they have a much shorter adaptation period. However, in some cases with soft contacts, vision may not be as sharp as with RGP contacts.

When prescribing a soft contact, my first lens of choice is CibaVision?s O2Optix?. This lens can be worn for up to one week extended wear or two weeks daily wear. Oasys? with Hydraclear, which is primarily targeted for patients who experience contact lens dryness.

Eyelid surgery can correct this by removing extraneous fat and trimming sagging skin and muscle tissue. Once the incisions have healed and the swelling has gone down, eyes look rejuvenated, more alert, and youthful.

Selecting a reputable plastic surgeon is critical to a successful surgery. The surgeon you select should be Board Certified and have considerable experience with blepharoplasty. Once you have selected a surgeon, the initial consultation is very important. Make sure to bring detailed copies of your medical records, since the doctor will need to complete a medical history. The doctor will perform a close examination of your eyes and eyelids, including a vision exam, as well as discuss your goals for the surgery. Your surgeon may also give you a sedative, either orally or intravenously, to help relax you since patients are kept awake during the procedure. Under local anesthesia, you will not feel any pain but may experience some tugging or pressure during the surgery. In certain circumstances, some surgeons opt to perform blepharoplasty under a general anesthetic, in which case the patient is asleep throughout the entire procedure.

The surgeries typically last from 90 minutes to 3 hours, depending on how many eyelids are to be corrected. During the procedure, the surgeon makes small incisions along the natural lines of the eyelids ? just under the eyelashes on the lower lids and in the deep creases of your upper eyelids. Excess fat is removed through the incision, and sagging muscle and skin are trimmed to neaten your appearance. Once the trimming is completed, your surgeon will use tiny sutures to stitch up the incision.

Once the surgery is complete, your surgeon will advise you on how to care for the incision. You will likely to be told to keep your eyes lubricated with an antibiotic ointment and take pain medication to control any discomfort you feel during the healing process. Cold compresses can be used to minimize swelling and bruising although patients will find that even with the compresses the swelling, tenderness, and bruising will be present for several days and may even last up to a month. Your eyes may tingle and feel gummy or dry for several days as well. Expect to return for a follow-up visit to your surgeon a few days after the procedure for a checkup and removal of your stitches.

Remember that the healing process takes time and it may be several weeks before you are completely recovered. In time the incisions will become less and less noticeable, fading into a thin white line that will be barely noticeable. The end result of the surgery though, should be brighter more alert eyes that make you look well-rested and youthful.

You can use other extensions (I've even used the ".to" extension on occasion) if necessary, just keep in mind that you will need to put forth a little extra marketing effort to promote the website address as people typically assume a .com extension as the norm. Whatever you do, don't use a domain name that is a confusing amalgam of letters and numbers that is hard to remember and even harder for your customer to type in.

One good way to approach the task of naming a business is to do so from your customer's point of view. Your business name should clearly define your offering and communicate your message to customers. Put yourself in your customer's shoes for a moment. If you were looking for a business that provides your product or service, what would you expect that business to be called? If you were in the market for computer parts, for example, wouldn't you look for a business that has "computer parts" reflected in the business name? Jim's Computer Parts may not sound as snazzy as Jim's Electronics Emporium, but snazzy doesn't pay the bills. Happy customers who quickly identify you as the source of their purchase do.

The name of your business can also spark subconscious reactions in a customer that may drive them to you or drive them away. Words like quality, complete, executive, best, low-cost, and on time often spark positive reactions in the mind of the consumer. Words like cheap, discount, and used tend to create negative emotions. You'll notice that no one claims to sell used cars anymore, but the dealer lots are loaded with vehicles that are "previously owned."

Finally, let's talk about things to avoid. Experts agree that you should avoid using generic terms like enterprise, corporation, partners, and unlimited as part of your everyday business name. These terms are fine for the legal business entity name, but are often too unclear for everyday use. Can you tell me what any of these companies do: ABC Corporation, Big Dog Enterprises, M&B Partners, and Discounts Unlimited sell? I didn't think so.

Also avoid abstract names like Yahoo, Google, Monster and Flip Dog (I am not going to list the names of the numerous local high tech firms that have bucked this rule :o). Abstract names will require a subtitle to explain what the business does or an expensive marketing campaign that brands the name into the minds of consumers. Unless you have deep pockets, I suggest you go with a name that describes your business at first glance and leave abstraction to the likes of Cher.

Finally, you should avoid hokey names, unless of course, you are starting a hokey business. Crazy Dave's Stereo Shop is a great name if the business is really run by Crazy Dave and his personality is exploited in the marketing of the business.

However, if you want to be taken serious, then give your business a serious name.

Would you go to Crazy Dave's House of Dentistry?

Neither would I.

Eye wear may be a fashion accessory, but when you depend on it for seeing the world, it?s a lot more. Today, many people are turning to refractive laser eye surgery, such as Lasik surgery, to improve their vision. However, this high-cost surgery is normally not paid for by insurance because it fails to meet the conservative therapy test and is classified as cosmetic. This trend is shifting.

We are seeing an increasing number of circumstances where refractive eye surgery is determined to be medically necessary and, therefore, payable by the insurance provider. For example, if conservative therapy treatments fail or if a patient is intolerant of contact lenses and cannot wear glasses because they might pose a risk in their job (e.g. police or firefighters), refractive eye surgery may be the most medically viable treatment option.

Conservative Therapy Options:

Examples of conservative therapy treatments include the following:

Photorefractive keratectomy (PRK) ? a refractive laser surgical procedure that is often done (and covered) for a patient that suffers from recurrent corneal erosion (e.g. diabetics). With PRK, a laser is used to remove corneal tissue to correct vision problems.

Patching, epithelial debridement (mechanical removal of faulty tissue), and bandaged contact lens placement ? treatments that often don?t work.

Stromal puncturing ? a procedure involving the physician puncturing the anterior corneal stroma ? can also be used but can fail or result in permanent vision loss. In such cases, PRK is the best alternative for removing the damaged cells (or membranes) and possibly correcting the vision problems.

Physicians can help patients who are in medical need of laser eye surgery make a stronger case by initiating the following:

Document a full medical history of the patient?s eye complaints

Show proof that the patient has not responded to conservative treatments and that the purpose of the surgery is for medical reasons other than simply improved eyesight

Still, even when the physician does this, each decision is taken on a case-by-case basis and is subject to the extenuating circumstances clearly noted in the patient?s medical record.
